My response to Dr. Mohta’s queries follows:

1. Washing of hands before the procedure is mandatory. Handling of the tube is from outside. Expressed Breastmilk flows from disposable syringe to the tube in a sterile conduit.

2. Strapping at the upper margin of the areola with a longitudinal strip of plaster keeps the tip in place.

3. There are two orifices in the infant feeding tube. Th orifice near the nipple is kept open and the one behind it is occluded. This does not come in the way of cleanliness.

4. This technique is for "NICU graduates" and "difficult to latch on babies" in a hospital setting. The strapping is done by the health worker. The pushing of the EBM is done by the mother.

5. Consent was given by the mother (and her husband) for publication in a scientific journal. The eyes were not covered to show the reaction of the mother, when the baby started sucking.
