Hematuria_Sole Manifestation of Hemorrhagic Disease of Newborn |
A term male baby (birth weight 3140 g) was born to
a healthy mother by normal vaginal delivery. Apgar score was normal. The baby was
exclusively breast fed. After 58 hours, he developed gross hematuria, but had no bleeding
from elsewhere. He was active and pink, anterior fontanel was at level, had no umbilical
or cutaneous bleeds or organomegaly. Bladder was not distended. Urine examination revealed
albumin 1-2 +, pus cells 1-2/HPF, RBCs - field packed. Hemoglobin was 12.8 g/dl, platelets
were 1,80,000/cu mm, PT was 25 sec (control-17 sec), creatinine was 0.7 mg/dl. Ultrasound
scan abdomen was normal. The infant responded within 12 hours to an intramuscular
injection of Vitamin K - 1 mg, with complete resolution of hematuria. Classical hemorrhagic disease of newborn occurs in 0.1% of live births, usually presenting with umbilical or grastointestinal hemorr-hage(1,2). Here we report a neonate with hematuria as the sole manifestation of classical HDN. One should entertain this possibility also whenever confronted with isolated hematuria in an otherwise healthy newborn. Gurmeet Kaur, References 1.
