New lact-aid Technique Queries

I read with interest "A New Lact-aid Technique" by Benakappa(1). I have following observations to make on the method described.

1. It is definitely not possible to keep the method hygienic if it entails strapping/taping of the tube to the chest wall of the mother from clavicle to nipple.

2. It is very difficult to maintain the tip of the feeding tube close to the nipple of the mother unless it is fixed to be the nipple itself with a tape. This is going to be uncomfortable to the mother and unhygienic.

3. It is not clear which proximal orifice of the feeding tube author plans to occlude. If it is the opening near the distal end, taping it everytime for blocking will hinder maintenance of the cleanliness.

4. I can foresee the problems which a mother will face if she has to strap the tube to her breast, maintain its position at nipple, push expressed breast milk through it and support the baby against the breast all alone.

5. I wonder whether the author took consent of the mother to publish the photograph without hiding the identity. If not, does it not breach her privacy?
